how would you write a source card for an e-book?

I need to have 5 sources for a research paper, problem is, one of my sources is an e-book and I am kind of insecure and unsure of how to write a source card for the e-book.
Would it be the same as writting a source card for any other book?…

using MLA format (the most common format) an online book is written as such:
last name, first name. title underlined. city or state published, date published. date acessed .

an example, including formatting:
Douglass, Frederick. My ******* and My Freedom (underline). Boston, 1855. 8 June 1999 (date acessed) .
